To submit a claim, provide us with the following information and we will contact you with the aim to settle your claim within 48 hours of receiving all the required documentation. You can view the Mobilife Claim Management Policy here

Please note you should be able to provide at least the following information:

  • Name, Identity document and bank account details of the claimant
  • Certified copy of the death certificate
  • Form BI - 1680
  • Form BI - 14 : Burial order
  • Certified copy of the deceased's identity document
  • Marriage certificate or birth certificate (if you are the owner claiming for an additional life covered on your Funeral Plan)
